I hereby authorize Bank Muamalat Malaysia Berhad to conduct credit checks with the Credit Tip-Off System (CTOS) and/or any other credit reference agencies or any background/reference check from my current/former employers for a period up to seven (7) years from the date of my employment application.
I further consent and authorized any individual, firm, corporation, consumer or credit reporting agency, or government agency to release or provide any information within their knowledge or record pertaining to me, and I agree to hold any or all of the blamless and free of any liability for releasing any truthful information that is within their knowledge or records. I also agree to hold the Company blameless and free of any liability for using any information received from such partiesin making an employment decision regarding me.
I have not been convicted within or outside Malaysia of any offence in particular but not limited to acts involving fraud, dishonesty, the taking or possession of prohibited drugs or any legal proceedings pending or threatened against me for such offence.
I have not had any disciplinary proceedings taken against me or suspended from work for any reason, in particular but not limited to acts involving fraud, dishonesty or abuse or in possession of any prohibited drugs, by any of my previous employers, nor have I resigned from any of my previous employments to avoid disciplinary proceedings or dismissal for any reason, including fraud, dishonesty or, the taking of possession of prohibited drugs.
I agree that the Bank shall not be liable for any losses, costs and expenses that may be incurred by myself arising from the Bank's decision to reject my application.
I agree that the Bank shall not be liable to reimburse any expenses incurred by myself for the purpose of this recruitment, in the event my application is rejected.
I hereby agree that all the information I have stated in my job application is the truth and where I have misrepresented any information which is subsequently discovered by the company, I will be liable to disciplinary action, notwithstanding my dismissal from service. Furthermore, I agree to repay the Bank for all salaries and benefits I earned during my employment with the organisation in the event of any misrepresentation and/or omission of facts.
I hereby declare that I have made full and true disclosure of all material facts and information to the best of my knowledge and have not concealed or omitted any material fact or information and I fully understand that if any of the declaration, material facts or information provided above are found to be incorrect, incomplete or untrue, the Bank reserves its absolute right to terminate my employment without notice or compensation.
I hereby undertake to inform Bank Muamalat Malaysia Berhad of any new development that may arise in relation to the information and declaration provided above.
I hereby consent Bank Muamalat Malaysia Berhad to share this application within DRB-HICOM Group and Subsidiary/Associate Companies should there any position available and suitable to me.
